In which cases is it necessary to perform PSR?

  • during first dental visit;
  • patients during follow-up visits with previously recorded PSR codes «0», «1», «2».

How to use online PSR tool?

First step (entering patient's data)

On the first page of PSR online tool enter the information about patient (name, age, gender, smoking) and name of dental professional. Click «Upload logo» to add logo of your clinic and personalize form with examination results. Click «Continue» to start entering examination data.

Second step (entering PSR data)

To create PSR you need to fill the cells of respecitve sextants with the highest code score or “*” (if present) obtained during periodontal examination.

When you set the cursor in the table cell, the teeth of the respective sextant will be painted in blue (active sexstant). After entering data into a cell of the respective sextant, the teeth in this section will be painted white. Sextants that are not filled yet are marked in grey. Highest score should be entered into the cell of respective sextant after examination of all teeth in the sextant (at least two teeth). In other words, the first recorded code "4" in sextant allows you to stop the examination of this sextant and move to next one.

If there is only one tooth in sextant, it has to be examined, information has to be entered into the cell of next sextant and present sextant should be marked as edentulous.

Edentulous sextant should be marked as «—». «*» indicates furcation involvement, gingival recession more than 3 mm and tooth mobility.

What are the PSR evaluation criteria?

The main criteria (codes «0», «1», «2», «3», «4»):

  • probing depth;
  • bleeding on probing;
  • calculus and defective margins on restorations are present.

Periodontal abnormalities such as: (code «*»):

  • furcation involvement,
  • gingival recession more than 3 mm,
  • tooth mobility.

Which instrument should be used to perform PSR?

Black (colored) band of periodontal probe marked at 2 mm (edges of marks correspond to lenghts of 3.5 and 5.5 mm) is quick and precise indicator of probing depths.

How many points do you need to probe each tooth to evaluate PSR?

In order to precisely evaluate probing depth it is extremely important to performe six-point probing. From occlusal point of view you can divide a tooth in 6 parts. The highest score should be entered into a cell.

Which inclination of the periodontal probe should be to perform PSR accurately?

The probe should be “walked around” the sulcus/pockets in each sextant. Periodontal probe should have inclination in mesio-distal direction (A and B, respectively) and to be parallel to longitudinal tooth axis.

What is probing depth?

Probing depth – the measured depth from the free end of the gingival margin (GM) to to the bottom of periodontal pocket (PB).

Code «1»

✓ color-coded reference mark is completely visible;
✓ bleeding on probing;
✓ no calculus or defective margins on restorations are present.

✓ color-coded reference mark is completely visible;
✓ bleeding on probing;
✓ calculus or defective margins on restorations are present.

Code «3»

✓ color-coded reference mark is partially visible ➔ probing depth 3,5-5,5 mm

Code «4»

✓ color-coded reference mark is not visible ➔ probing depth more than 6 мм

Code «*»

The * symbol is added to the code of a sextant exhibiting any of the following abnormalities:

  • furcation involvement
  • gingival recession more than 3 mm
  • mobility.

  • appropriate preventive care and oral care skills control

Code «1»

  • oral care training and instructions;
  • professional oral hygiene.

Code «2»

  • oral care training and instructions;
  • professional oral hygiene;
  • removal of plaque retentive factors.

Code «3»

  • scaling and root planing of all affected areas;
  • refferal to a specialist may be indicated.

Code «4»

  • complex periodontal treatment;
  • refferal to a specialist.

Code «*»

  • complex periodontal treatment;
  • refferal to a specialist.

How to save results of online PSR tool?

Click “Continue” after entering all obtained infromation to proceed and watch the results. Click “Print” or “Save” icon to print or save (in PDF) the results both for doctor and patient. You can also choose an option “To print all versions simultaneosly”.
